STAFF/STUDENT ACCIDENTS

ACCIDENT REPORTING:  The following procedure is to be used to report all accidents and a copy of the report for it is attached for reference:
a. All staff members are to report all accidents and injuries to the school nurse and the school administration by the fastest available means.
b. The Administration will file telephone reports with the Superintendent and the Business Administrator as soon as practical.
c. The school nurse will provide the staff member with an accident report form which must be completed and returned to the nurse within 48 hours of the accident.
d. The school nurse is responsible for providing the district office a completed copy of the accident report within 72 hours of the incident.
e. The nurse will send the Principal the names of all teachers who have outstanding accident report forms after the 48 hours have elapsed and the Principal will be responsible for obtaining them from the teacher.

CONTACT LENS WEARING:  The following procedure applies to the wearing of contact lens in Industrial Arts shops, Physical Education and Science Laboratories:
a. Teachers of classes in which eye protection is required as well as physical education classes will note the letters "CL" next to the name of any student in the class who wears contact lenses in their grade book or class roster. This notation will be verified by the department supervisors.
b. The wearing of contact lenses is not specifically prohibited in shop or physical education classes, but it is strongly recommended that it not be done as the possibility for loss or damage of the lens as well as the possibility of injury is high.
c. The wearing of contact lenses in science laboratories may be prohibited by the teacher at any time through the simple process of an announcement to the class. This discretion is given to the teacher because the nature of some chemicals is such that the fumes may be damaging to the lenses. It is the teacher's responsibility to know the dangers inherent in the use of any chemicals in class and adjust the student's dress accordingly, including the prohibition of contact lenses.

INSURANCE COVERAGE:  The insurance carrier will not cover claims resulting from recreational play by coaches with pupils. Play with students is only covered if it is an integral part of the instructional program such as a demonstration of a technique using a student as part of the demonstration.

PLASTICS PROHIBITED:  The use of plastics in industrial arts programs is prohibited without the use of proper ventilation systems which currently do not exist in the schools in this district.
